While this decision was not made lightly, we want to assure you that your well-being remains our top priority. We are here for you during this transition and will be available for any emergency care you may need. Should you require ongoing dental care, we strongly encourage you to continue with another trusted provider to maintain your dental health. If you need assistance in finding a new provider, please do not hesitate to reach out—we would be happy to help you in any way we can.

We are more than happy to forward your records to your new provider once we receive a written release from you.
